Prótese ou não? A resposta não está em uma regra, mas no resultado que você deseja.

Implant or not? The answer lies in the result you desire.

Implant or not? The answer isn’t in a rule, but in the result you desire.

Mastopexy can be performed with or without implants, and each choice brings different characteristics. An implant can provide more projection and definition to the décolletage, while the technique without an implant preserves a more natural result and eliminates issues related to the presence of a prosthesis in the future.

The most important thing is to understand that there is no better option for everyone. There is the one that makes sense for your body, your expectations, and the surgical planning.

In many consultations here in the office, I hear a doubt: should I have my mastopexy or my surgery with or without an implant? Which is better? This question can have several answers, because what is better involves several aspects. So what I like to always tell patients is that there are advantages to doing a mastopexy and placing an implant, and also advantages to doing the mastopexy without placing an implant. So here we have to understand that the implant is an implant, it is a medical device that has been very well studied to be used for both reconstructive and aesthetic purposes, but it is a foreign body. What will our organism do with the implant when it is inside? It will form a capsule, a scar around it, and it will stay quiet there. For how long? It can be countless years and it will stay quiet all this time. But when you have a foreign body in your organism, your organism may not accept it either, so you may have some capsular contracture, which is a hardening of this capsule. At some point, due to material wear, microtraumas of the area, you may have a rupture of this implant, or in a more acute way, you may have some imbalances there that cause the organism to create inflammation around this implant and expel this implant, and that you have to remove this prosthesis. So, these would be the disadvantages of having a prosthesis. But the advantages are many, because when you do a mastopexy with a prosthesis, for you to have a stable breast shape, so that you can get that more projected and firmer décolletage in the medium or long term, it is with the prosthesis. A mastopexy without a prosthesis, the most we do is all the assembly and try to structure it so that it has the maximum possible projection in the décolletage, still it will be a consistency of natural mammary tissue. This prosthesis consistency you will only acquire with the prosthesis. So, there are many cases where the patient really seeks this more marked, more stable décolletage in the medium and long term, that she will put on a decote and that larger projection will remain, then the mastopexy with prosthesis is the most indicated for her. So, it is very much a question of conversation about the patient’s expectations and also the future. If she is interested in, for example, in 15, 20 years, having to deal with a new surgery to do an exchange, a removal of this prosthesis, or if she prefers something that in the long term will not have this maintenance. So, an individualized conversation, every choice has its pros and cons and it is a choice that we make together between the doctor and the patient.
